Life Skills Coach - Community Inclusion Program (Part Time) - North Jersey Friendship House

About the Job
Primary Function: Support and assist individuals with developmental disabilities in educational, enrichment or recreational activities that are intended to enhance inclusion in the community
Hours: 20 Hours a week (Monday - Thursday - 3-8 pm)
Essential Functions:
- Work with individuals in small groups: to include recreational, leisure and educational activities in the community.
- Prepare for and assist with community integration and social skills planning
- Work with Program Coordinator in the development of curriculum and activity planning
- Support and teach pro-social skills to the participants to ensure and reinforce skill development
- Evaluate participant satisfaction with activities and make recommendations as needed
- Develop rapport with participants and form a trusting relationship with them
- Provide participants with transportation to and from activities as needed or assigned by Program Coordinator
- Ensure participants safety in the community
- Complete a daily activity note for each Community Inclusion participant in your group noting progress, needs, special skills and preferences in their electronic health record
- Work with Program Coordinator in the development of annual goals and strategies to meet their stated goals
Requirements:
- Bachelor's or Associates degree (preferred) from an accredited college in the Human Services field or one (1) year of related experience
- Experience in working with individuals with disabilities
- Ability to work well in a team setting
- Valid driver's license with a clean record
