We are seeking a Life Skills Coordinator to join our team in our Montgomery County, Colmar, PA residential location.
The Life Skills Coordinator actively participates in individual assessments and development of individual service plan and goal setting with clinical liaison and supervisor. They educate the direct care staff on how to provide rehabilitation services that correlate to the goals identified on the service plan and how to appropriately document such services at our 10 Resident Community Residential Rehabilitation facility (CRR) in Colmar, PA.
The Life Skills Coordinator will be responsible for:
* Teaching and Instruction of residents and staff
* Encouraging positive change
* Skill demonstration and practice
* Promoting individual wellness, self-management, and recovery
* Assisting in the restoration and/or improvement of self-care and social skills
* Facilitating effective strategies for strengthened independent living and the achievement of self-sufficiency goals
* Exposing individuals to academic, vocational and employment training and resources
* Providing individual services to those who cannot have their needs met by group activities
* Facilitating culturally relevant activities directed toward improved social relationships
* Creates and implements the daily schedule of activities and engages residents in those activities
* Creates and provides curriculum that will help individuals learn to become active in the community, gaining independence in use of public transportation, budgeting, shopping, and accessing social, spiritual, and recreational outlets
* Completes daily documentation of individual progress and curriculum/skills used in programming
* Builds relationships and communicates with Human Services providers and other community referral sources
* Provides Supervisor with up-to-date information regarding on-going programmatic, quality, authorization, and compliance issues
* Making consumer discharge recommendations and summaries at times
* Complete training as assigned by supervisor focusing on skills teaching and psychiatric rehabilitation principles, in addition to the training required for all staff working in a CRR
* Other duties as assigned, as it relates to the position
Earn $22.37 per hour.

The ideal candidate will possess the following qualifications:
* History of demonstrated therapy in support of treatment plans.
* Demonstrated ability to manage in a crisis.
* Proven ability to establish good boundaries.
* Ability to work effectively across interdepartmentally.
* Ability to represent the organization appropriately to visitors, etc.
* Ability to manage multiple priorities.
* Ability to provide care in a manner that considers individuals rights, comfort, and overall therapeutic environment.
* Intermediate skillset with computers
* Bachelor's degree in a human services field required.
* CPRP certification is a PLUS!
* 1 year experience working in the Behavioral Health field/ population in a group home or a residential facility setting preferred.
* Must have a valid and clean driver’s license and clean driving record.
* Shift can vary based on client's needs, primarily 10-6 or 11-7with a week-end rotation. Over-time available.
