Job Description:

• Provide exceptional home health physical therapy services to patients in accordance with individualized care plans.
• Perform thorough patient assessments and develop effective treatment strategies to address mobility, strength, and functional limitations.
• Administer therapeutic exercises, manual techniques, and modalities to achieve optimal patient outcomes.
• Collaborate closely with healthcare teams to coordinate care and ensure continuity of treatment.
• Maintain accurate documentation of patient progress and communication with healthcare providers.

Education and Certification Requirements:

• Ohio Physical Therapy License.
• Minimum of 1 year of experience in physical therapy.

Contract Details:

• Contract Length: 26 weeks
• Weekly Compensation: Up to $2,700 based on experience
