Job title - Heart Transplant Social Worker Location - Boston, MA, 02115 Duration - 13 weeks(high possibility of extension)Shift - 08:00 - 16:30 Pay - $65/hr Responsibilities/ Additional Information - In this role, you will provide support, advocacy and guidance to transplant patients and their families throughout the transplant process. You will be responsible for completing comprehensive biopsychosocial assessments, participating in Selection Committee and collaborating with the transplant team to ensure coordinated and integrated care delivery. Experience in transplant or organ donation preferred.Licenses and CertificationMust hold current SW license in MA.
