Job Title: Medical social worker - Home CareLocation: Newton, MA, 02459Assignment Length: 13 weeks Shift: 08:00 - 16:30Hourly Pay Rate: $65/hrJob description:MUST HAVE 1 year Home Care experience1 year of social work experienceCurrent licensure in the state of MassachusettsReliable, independent means of transportation to make home visits. Must be willing to work weekends/holidays/on call in accordance with department need/rotation. Weekend rotations are once every 6 weeks. Responsibilities: Provide and coordinate social services for patients from birth to over 100 years in their homes. Advocating for patients who are working through psychological, health, family, and financial struggles. Assessing the mental and emotional stability of the patient by understanding the social and financial condition of the patient along with their family. Coordinate and collaborate with health-care workers to provide optimal health-care services to the patient. Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures.
