**License:** Active CT license

**Certifications:**
- Board Certified (BC)
- ONS chemotherapy certification
- Basic Life Support (BLS)
- Advanced Cardiovascular Life Support (ACLS)
- DEA certification

**Must-Have:**
- Epic proficient
- Independent practitioner capabilities

**Job Description:**
- Independently evaluate adult medicine patients
- Document care plans
- Conduct some new patient evaluations in benign and malignant hematology
- Manage chemotherapy toxicities
- Provide support to nurses (RNs), medical assistants (MA), and certified oncology assistants (COA)
- Perform chemotherapy prescribing with certification
