Practice Setting Office/Clinic Outpatient/Inpatient/Free-Standing Surgical Center: Office/clinicDates of Coverage: ASAP flexible Schedule Shifts and Hours: 8a-12p 1-5p Call Schedule: NA Call Ratio: NA Average Number of Patients Per Day - Clinic: 25-30 per 8 hour day Average Number of Patients Per Day - Hospital: NA Number of Physicians in Practice: one part time dermatologist Will Other Dermatologists Provide Coverage During Locums Coverage Dates? Yes What Percentage of the Practice is Pediatric Adolescent Adult Geriatric: see all ages. What Percentage of the Practice is Pediatric Adolescent Adult Geriatric: see all ages. Describe Office Ancillary Staff e.g. Number of RN’s LPN’s MA’s etc.: 2 MAs plus LPN support derm. Additional staff in office to coverDocumentation System/EMR: meditech expanse Required Surgical Procedures: Biopsy Excision Electrodesiccation Dermabrasion Estimated Credentialing Timeframe: 1 week after all documents submitted
