Job Related Details: • In home health assessments for medicare population. • 10 hr shifts (this includes time allotted for admin work) • Patient Census: Expected to be 6-8 per day (45-60 min visits) Schedule:• Mon – Fri, 10 hr shifts • Min 32 hrs/wk, up to 40 hrs/wk• Exact schedule to be determined during interview and prior to confirmationProvider Requirements:• Must hold an active license in the state provider is being presented for. • Board Certification from a nationally recognized certifying body for Nurse Practitioners required• Min of two years of working experience preferred. • Experience and understanding of population health management preferred• Must be willing and available to work the schedule mentioned above, for the duration of the assignment• Must have experience with billing codes, CPT codes and are detailed and thorough with patient charting• Must have reliable vehicle and be comfortable with maps and following directions• Experience with in home care is a plus
