About The Position
**Job Title:** Registered Nurse (RN) **Job Specialty:** Long-Term Care (LTC) Certified Wound, Ostomy, and Continence Nurse (CWON) **Job Duration:** 13 weeks **Shift:** Night - 5x8 - 11PM-7:30AM **Guaranteed Hours:** 40 **Experience:** Minimum 2 years in a long-term care or subacute rehab facility **License:** NJ State Professional Registered Nurse license or multi-state compact license **Certifications:** BLS - AHA **Must-Have:** Ability to demonstrate a team approach to problem-solving with excellent communication and collaboration skills **Job Description:** - Work as part of a multidisciplinary team to deliver patient-centered nursing and rehabilitation services. - Support the Unit Manager to maintain high clinical standards and patient experience. - Ensure all assigned residents receive medications, treatments, and nursing care as ordered. - Complete all required documentation according to state and federal guidelines. - Supervise and assign tasks to nursing personnel on the unit. - Establish nursing care plans utilizing nursing diagnoses in cooperation with supervisors and other nursing personnel. - Implement plans of care for residents. - Prioritize and complete routine assignments accurately and independently within guidelines and schedules. - Recognize psychosocial needs and provide emotional support as necessary. - Maintain accurate and informative records on all patients. - Identify and report real or potential problems in patient care development or administration. - Maintain adequate inventory of medications, equipment, and supplies. - Record, report, and interpret patient responses to treatment and care to the inter-disciplinary team. - Identify and assess changes in patient conditions and report to the charge nurse. - Maintain equipment per facility policy and consistent with specifications. - Participate and represent nursing staff on committees as requested by supervisors. - Identify patient needs and opportunities for treatment, providing services to meet those needs continuously. - Maintain professional competence and stay current with occupational health and nursing trends through continuing education and training. - Foster good relationships and communications with team members, patients, families, and visitors. - Assist with new team member orientation and provide essential experiences to prepare them for independent job performance. - Report problems promptly to the manager. - Float as required, work every other weekend, and fulfill holiday requirements.
