Responsibilities/Duties:
- Maintains nursing documentation and medication records.
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR .
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc.
- Assesses residents’ health and emotional needs.
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director.
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas.
- Other duties as assigned, but not limited to: provides comprehensive hourly nursing care in individuals homes or community-based residential settings, provides G-tube care/feeding, administering medications, completing dysphagia assessments, monitors program participant vitals, assists with implementing PT/OT regimens, completing of Health Risk Screening Tool (HRST) clinical reviews, managing medical equipment and technology and completing patient care assessments & other assigned documentation.
Required Experience:
- Licensed Practical Nurse with current license
Required Education:
- Licensed Practical Nurse with current license
Required Qualifications:
- Licensed Practical Nurse with current license
- Valid Drivers License
