LPN ASSESSMENT NURSE
Are you an experienced LPN with exceptional nursing skills and enjoys working with patients in a home setting? We are looking for individuals that have experience providing the following nursing services:
- Perform Health Assessments
- Schedule and perform Supervisory Visits to other skilled nurses (RN/LPNs), certified nurse assistants (CNAs) and personal care aides (PCAs)
- Provide Care Coordination
Job Description:
Responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services will be performed according to compliance with state and local laws/ regulations, the Nurse Practice Act and all other healthcare accreditation standards.
- Initiates and identifies client's needs through assessment
- Provides nursing services based on the plan of care
- Prepares necessary items for treatment and procedures
- Implements and uses current standards to prevent spread of infection, instructs and may prepare medications as appropriate for the patient's care and safety
- Makes treatment recommendations based upon the patient's acuity level and goal potential
- Excels at the admissions process including same-day electronic documentation
- Flexible to accomplish admission with time, travel time or geographic constraints
- This clinician provides and documents skilled care on all visits
- Documentation skills must meet agency standards
- Position requires driving throughout day
Other Requirements:
- Able to follow regulation guidelines and deadlines with submission of clinical paperwork
- Computer skills are a must, typing skills for reporting is needed and to update documentation in online portal
- Demonstrates willingness and ability to work with electronic input and telecommunication devices
- Organized and timely with clinical paperwork
- Excellent clinical skills and demonstrated ability to manage time well
- Excellent communication skills; must be able to communicate in a manner that is understandable to the staff and general public verbally and in writing
