Job Title: LPN Care Coordinator (1099 Contract)

Overview: We are seeking an experienced LPN Care Coordinator to join our team on a 1099 contract basis, depending on business needs. In this role, you will provide care management and coordination services directly to patients, ensuring their healthcare needs are met through effective communication and care plan development.

Key Responsibilities:

• Review patient EMR records and determine eligibility for care management services.
• Coordinate with Primary Care and Specialty Providers to avoid duplication of services.
• Educate providers and patients about available services and support chronic condition management.
• Develop and maintain individualized care plans, updating after each patient interaction.
• Monitor patient progress, ensure timely communication with care teams, and maintain thorough documentation.
• Conduct bi-monthly care calls to address patient needs and connect them to necessary community resources.
• Adhere to policies and procedures, including Medicare and HIPAA compliance.

Qualifications:

• Minimum of 5 years’ experience in patient-centered care.
• Proficiency with EMR systems and technology.
• Strong communication skills and attention to detail.
• LPN Compact License (or ability to obtain if not within a compact state).

Contract Type:
1099 contract, hours based on business needs.
