Summary: The LPN is responsible for the overall case management, and medication supervision of the residents, including personal care services in the Assisted Living Community.. Responsibilities include, but not limited to accurate transcribing orders, working with residents doctors and pharmacies to ensure the MAR's are up to date at all times, medication ordering and procurement, hiring and training of the med techs and personal care aids; annual review and ongoing coaching and counseling of the staff members: completion of case management notes, staffing schedule and submitting payroll hours. Ensure that the universal worker concept is implemented.
This is a hands-on position where assisting residents with activities of daily living, medication administration and acting as the charge nurse on the resident floors will be required. As the leader of the resident care services is expected to develop and maintain a positive atmosphere for residents, families, visitors and staff.
Responsibilities
- Accurate transcribing orders, working with resident doctors and pharmacies to ensure the MAR's are up to date at all times
- Dispense medication to residents as per the Medication Administration Record
- Medication ordering and procurement
- Supervision of the med techs and personal care aids;
- Ensure staffing levels and scheduling are in line with resident census levels
- Updating case management notes;
- Coordinate care with residents, family members and physicians
- As the leader of the resident care services is expected to develop and maintain a positive atmosphere for residents, families, visitors and staff
- Cover staffing shortages as needed
- Ensure staff is properly trained and certified
- On-call 24/7 hours for Emergencies and Manager on Duty responsibilities.
- Other duties as may be required or necessary
Qualifications:
- LPN with One (1) year experience in long term care or assisted living
- Current state license as an LPN in New York
- Ability to handle multiple priorities
- Able to supervise and work part of a team
- Excellent verbal and written communication skills
- Has good organizational and time management skills
- Experience with seniors preferred
- Able to make responsible choices and decisions and act in the resident’s best interest
- Ability to lift 25-35 pounds
