Reports To: Registered Nurse (RN) Supervisor
Job Summary:
The Licensed Practical Nurse (LPN) provides compassionate, high-quality care to elderly and disabled clients. Working closely with the RN Supervisor and other healthcare staff, the LPN performs essential patient care duties, monitors client health, administers medication, and supports each client’s plan of care. This role is integral to promoting client well-being, ensuring adherence to agency and state regulations, and upholding the mission of Care4Seniors to enhance the quality of life for our clients.
Key Responsibilities:
Direct Patient Care:
- Provide routine care such as checking vital signs, administering injections, dressing wounds, and other duties as per the client’s care plan.
- Assist clients with daily living activities (e.g., bathing, grooming, and feeding).
- Medication Administration:
- Administer prescribed medications and monitor for side effects or adverse reactions.
- Record and report any changes in client’s medication needs or reactions to medications.
- Client Health Monitoring:
- Conduct regular assessments of client’s health status and report findings to the RN Supervisor.
- Monitor for signs of infection, decline in health, or other issues that may need medical intervention.
- Documentation and Reporting:
- Maintain accurate and timely documentation in client records, including care provided, observations, and changes in client health.
- Communicate any concerns, updates, or incidents with the RN Supervisor and other appropriate staff.
- Support and Education:
- Educate clients and their families on proper health maintenance and care techniques.
- Provide emotional support to clients and their families, promoting a caring and supportive environment.
- Ensure all practices meet federal, state, and local standards, as well as Care4Seniors policies.
- Participate in training and ensure compliance with Electronic Visit Verification (EVV) and Medicaid requirements.
Qualifications:
- Education: Graduate of an accredited LPN program.
- Licensure: Valid LPN license in the state of Ohio.
- Experience: Prior experience in home care, senior care, or a healthcare setting is preferred.
- Strong communication and interpersonal skills.
- Compassionate, reliable, and detail-oriented.
- Ability to work independently and as part of a team.
- Proficient in documenting care in health records and familiarity with EVV systems is a plus.
