Hours: Full-time | Days 8:00am - 5:00pm | .8 FTE (32 hours) Fri/Sat/Sun/Mon OR 1.0 FTE (40 hours) Thurs/Fri/Sat/Sun/Mon

Territory:  We are looking for someone to be able to travel throughout Interim's service area based on client’s needs.

A few must-haves for this position: 

• Current license as a Licensed Practical Nurse (LPN) in the State of MN.  License is active and in good standing.
• 2 years' experience as an LPN.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
• CPR Certification 

Some functions of what you would do as an LPN:

• Participate in ongoing interdisciplinary assessment of the patient.
• Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides services that are ordered by the physician as indicated in the plan of care.
• Recognizes and reports changes in the patient’s condition to the RN supervisor.
• Counsels and educates the patient and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing patient’s progress.
