JOB SUMMARY
Provides quality care that is dependent on physician order or directed by a Registered Nurse and that meets facility requirements and local, state and federal regulations. The LPN delivers nursing interventions based on professional judgement in response to individualized resident needs. Promotes restorative health and/or supportive care using the nursing process to assess, plan, implement, and evaluate nursing care. Provides oversight and assists in the coordination, supervision and management of the nursing assistant.
QUALIFICATIONS
Current LPN license by the Mississippi Board of Nursing.
Must be able to read, write, and understand written and verbal communication.

PHYSICAL REQUIREMENTS
Ability to sit, stand, bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
Ability to lift and carry in excess of 50 pounds.
ESSENTIAL FUNCTIONS / ASSIGNED TASKS
• Provide resident care by collecting, monitoring, and reporting information and responding to basic needs
• Assist with the admission process by collecting resident and family information
• Promote resident engagement by considering specific needs, values and preferences
• Assist with resident and family education as necessary
• Provide staff education, consultation, and oversight based on facility policy, planned or identified needs, and regulatory guidance
• Accurately and completely document resident information and nursing care in the medical record
• Promote continuity of care by reporting activities, incidences, and changing resident needs or condition to the next caregiver
• Demonstrate infection prevention behaviors, consistent hand hygiene, and use of appropriate PPE
• Adhere to safe medication administration while preparing, administering, documenting and monitoring medications
• Recognize safety risk and implement prevention plan for falls, skin breakdown and injury
• Conduct rounds throughout the shift to direct the Nurse Assistant in the delivery of direct resident care
• Communicate with residents, family members, and other disciplines or healthcare professionals regarding the progress and status of the resident
• Monitor the Nursing Assistant’s documentation and provide feedback for correction
• Provide treatments as ordered following facility policy and clinical practice guidelines
• Demonstrate competence in performance of LPN clinical skills
• Perform other job duties and tasks as assigned
