Duties:

- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Perform specialized procedures such as gastric lavage, sterile processing, and dialysis.
- Conduct discharge planning and coordinate post-hospital care for patients.
- Provide care for patients with special needs, including those with physical or developmental disabilities.
- Administer treatments and therapies as prescribed by physicians.
- Document patient information accurately and maintain medical records using electronic health record systems such as Cerner and Athenahealth.
- Educate patients and their families on healthcare procedures, medications, and self-care techniques
