Position Summary
The Medical Coordinator works in conjunction with the program RN, where applicable, and site supervisors to assist in the coordination of the healthcare services for the program consumers. A commitment to the RHD values should be demonstrated as job duties are performed.
Essential Duties and Functions:
- Maintains nursing documentation and medication records.
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR.
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc.
- Assesses residents’ health and emotional needs.
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director.
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas.
- Other duties as assigned.
Healthcare Services:
- Accompanies staff at medical appointments to educate on reason for visit and to clarify orders as written by healthcare provider as necessary.
- Coordinates with hospital case management and assures information is shared, where applicable.
- Coordinates a team meeting with hospital case management within 24 hours of a consumers’ admission to ensure information regarding the consumer is shared with all relevant providers, maintains regulars contact (in person and by phone) during hospitalization, and coordinates and attends discharge planning meetings where applicable.
- Participates as a member of the multi-disciplinary team and in case conferences to provide information pertaining to the healthcare of program consumers.
- Partners with the Program Director/RN, and members of the consumers’ team to ensure that efforts are made to have documented information on file for Durable Health Power of Attorney/ Advanced Directives as appropriate.
- Coordinates with staff to ensure that all annual appointments and documentation are completed as required.
Medical Protocol, Documentation and Compliance:
- Reviews consumer medical books on a quarterly basis to ensure that proper healthcare documentation is in accordance with DPW Regulations.
- In conjunction with the Program Director/RN, assists in the development, training, implementation and follow up for medical protocols to best support program consumers.
- Acts as a medication administration trainer and ensures that all program staff are current in their annual training and practicum.
- Maintains a system to ensure that all consumers are compliant with annual healthcare appointments and notifies Program Director/RN of any compliance concerns.
Administration:
- Schedules initial and follow up appointments, and completes corresponding appointment consult paperwork.
- Ensures that all healthcare releases and consent forms are accurately completed and current to ensure the continuity of care for consumers.
- Tracks and notifies Program Director/RN of consumer refusal of appointments or any other challenges.
- Maintains Lifetime History record for all consumers and updates annually.
- Other duties as assigned.
Qualifications:
- Licensed Practical Nurse with current license.
