A prestigious homecare company is seeking LPN's to visit group homes in Philadelphia
Type: Temp or Temp to Perm
Schedule: 1st Shift RN MonFri, 9:00am05:00pm
Location: Hatboro, PA
Job Description:
3-4 house visits a week
Entering medical information into database / Reports / forms
Attend meetings the bulk are virtual via zoom or teams some in person at HQ or at the group home
There is a doctors office at HQ will work with doctor regarding individuals on caseload to coordinate care
Ensure residents have all necessary information for 'outside consults' appointments nurse will create appointment packets for house staff to take resident on appointments. On rare occasions a nurse may need to meet staff and resident at the appointment to coordinate care / procedure instructions. Note the nurse does NOT take resident to appointment they would just meet them there.
Training house staff on medical changes for residents, deliver meds to house and doing house checks of medications, wound care at house
Coordinate medical care taking off doctors orders for med changes , medical care, updating team
When individual in hospital follow them while there, coordinate hosp DC ensure safe DC back to group home, post hospital assessment for readmission / covid testing
Will have lap top and I phone to answer while on duty. DO NOT answer when off duty.
Works with 2nd shift nurse for the same region 1st and 2nd shifts overlap so they can work together for a period of time.
