LPN - Palliative - Crisp Regional Health System

Position Summary: Works as LPN under the direction of the Hospice and Palliative Care Director and in close coordination with the Palliative NP and Palliative Medical Director to achieve a palliative plan of care for the palliative patient.

Basic Qualifications:

• Education: Completion of an accredited LPN Program

Experience:Prefer at least one year of experience, possesses excellent communication skills with both patients and members of the medical team, has strong clinical assessment skills, and is able to work well with an electronic medical record system

• Licensure, Registrations & Certifications:

This job requires maintaining a GA LPN License

Essential Job Responsibilities:

• Processing new referrals
• Maintaining the schedule for Palliative NP
• Once a schedule is made, LPN will enter the encounters in Cerner in EMR.
• Enter proposals for narcotics in EMR for MD to sign
• Track when narcotic scripts are needed
• Make weekly telephone calls to palliative patients or families
• Answer any calls that palliative patients make to the office and be able to triage needs for Palliative NP.
• Organize the monthly palliative meeting, including scheduling and keeping notes
• Actively participate in team meetings
• Assist with ordering DME, (faxing the orders and following up on DME needs)
• Assist with obtaining Pas for meds when required
• Coordinate with social workers for social needs
• Perform other related job duties as assigned.

Standards of Performance:

• Prepares, completes and submits clinical documentation for all assigned patients within 48 hours.
• Reports and documents significant changes in the palliative patient to the palliative provider
• Provides interdisciplinary collaboration with all members of the palliative team
• Provides effective triage for the palliative patient and then reports abnormalities to the palliative provider
• Demonstrates positive interpersonal skills when communication with patients, families, coworkers, and providers
• Attends department in-service and keeps up to date on all required education, including CPR
• Demonstrates reliability as measured by compliance with CRHS time and attendance policies.
• Demonstrates adherence to CRHS Standards of Behavior as measured by education and attendance records, and maintenance of professional licensure.
• Demonstrates professional conduct by maintaining strict confidentiality, as measured by valid complaints given to the supervisor or director.
