About the Job
Our client, is an integrated residential treatment program developed to provide behavioral health and residential services to Community Behavioral Health consumers with serious mental illness, requiring extended comprehensive behavioral health support services in a non-hospital, communitybased residential setting is looking for an Licensed Practical Nurse
Type: Contract/ Temp (PRO-MARKET)/ Ongoing
Schedule: Full-Time, Monday to Friday 8am to 4pm.
Location: Phila, PA
ROLE AND RESPONSIBILITIES
Primarily responsible for the care of residents in all areas of treatment, to ensure their comfort and well-being.
Including, but not limited to, daily assessment of their physical and mental state, upkeep, and accuracy of their charts.
All work is performed in accordance with City, State and Federal regulations, policies and procedures, and professional mental health standards.
Brief outline of duties
Responsible for taking vital signs, assessment of resident's medical and psychological symptoms and the documentation of same in the nursing progress notes.
Ensure proper and accurate transcription of physician orders and that accurate documentation is maintained on the administration of medication, verbal physician orders and individuals that are on self-medication plans.
Inform the physician of acute or ongoing medical and/or psychiatric problems scheduled clinic appointment times and the results of medical assessments and testing.
Schedule outside medical appointments and ensure client's transportation.
Review client's health plan, develop and manage medical and medication goals for residents' comprehensive treatment plans including residents who are in the self-administration medication program.
Develop and run nursing groups, act as co-leader in group therapy and participate in treatment team meetings.
Act as liaison between the resident's physicians and the staff.
Provide training to residents and staff in the areas of hygiene, medication side effects, first aid, nutrition, etc.
Other duties as assigned by Associate Director.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
Current Pennsylvania LPN license
Experience working with Chronically mentally ill adults, preferably in a residential setting.
