LTSS Service Care Manager (LVN or Social Worker) - A-Line Staffing Solutions

A-Line staffing is hiring a Case Manager in Abilene, TX! This position has career growth potential with full time / 40+ hours per week.

*MUST have one of the following licenses: LVN or Social Worker OR a Bachelor Degree and 2-4 years of related case management experience*

Case Manager Details and Compensation:

• Location: 100% hybrid
• Payrate: $24 - $26 per hour PLUS mileage reimbursement
• Travel may be up to 2 hours one way to see a member in surrounding counties
• Required Availability: Monday - Friday 8am - 5pm - 3 days of visits and 2 days of documentation weekly

Case Manager Responsibilities:

• Conduct in-home face to face member assessments as needed and telephonic outreach
• Document assessments and contacts, develop and approve service plans, and ensure authorizations are completed.
• Assess member’s medical, safety and support service needs and coordinate the delivery of services. Provide education related to member’s health condition and care, medical consenter, or caregiver, Department of Family and Protective Services caseworker and family, if applicable, as well as the managing physician.
• Coordinate member care through collaboration with community outreach and member advocates, assigned service coordinators, and/or case managers.
• Educate providers and community resources on program components and available support services.
• Educate members to foster compliance with program and positively impact outcomes and 75% travel required.
• CM will have 3 days of visits and 2 days of documentation weekly.
• CM will have a caseload and work assigned tasks. CM will complete H2060 assessment.
• CM will fill put required forms for each assessment. CM will make follow up calls.

Case Manager Requirements:

• LVN or Social Worker OR a Bachelor Degree and 2-4 years of related case management experience
• 5+ years or more working with the aged, blind, or disabled population
• Experience with Social Service, Social Work, Care Management, or Services Coordination
• Bilingual in English and Spanish is highly preferred
