Job Specification:
As the LVN Case Manager, you will serve as a vital member of our interdisciplinary team, providing leadership, guidance, and support to ensure the delivery of high-quality case management services to residents. Responsibilities include:
- Overseeing and coordinating the day-to-day operations of the case management department, including staffing assignments, caseload management, and supervision of case management staff
- Conducting comprehensive assessments of residents' needs and developing individualized care plans in collaboration with interdisciplinary team members
- Providing counseling and support to residents and families to address psychosocial, emotional, and financial concerns, and connecting them with community resources and services as needed
- Advocating for residents' rights and preferences within the healthcare system, and facilitating communication between residents, families, and healthcare providers
- Ensuring compliance with state and federal regulations related to case management practice, confidentiality, and resident rights
- Participating in interdisciplinary team meetings, care conferences, and quality improvement initiatives to enhance resident care and outcomes
- Documenting all case management activities accurately and in accordance with regulatory standards
Qualifications:
- Holds a current LVN License in CA
- Experience in Clinical Management is prefered
- Proficient in managing multiple concurrent projects while maintaining an appropriate work pace to accommodate workload demands.
- Familiarity with regulations and requirements set forth by the Joint Commission, Centers for Medicare & Medicaid Services (CMS), California Department of Public Health (CDPH), and California Children's Services (CCS).
