About the Job
Job Summary:
- Education: Graduate of an accredited school of vocational/practical nursing. (Preferred) High School or GED Diploma.
- Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. (Preferred if applicable)
- Experience: One year of experience as a licensed vocational/practical nurse; home health experience preferred.
- Skills: Knowledge of medical terminology preferred. Good people skills.
- Transportation: Reliable transportation.
- Directs all daily referral and intake operations.
- Clinical review and appropriate documentation of new referrals to ensure each have all necessary documents for RCD, authorization and scheduling, orders, and requirements in a timely manner.
- Receives via telephone, e-mail, fax, Updox, Current EMR and appropriately and accurately documents new referrals in a timely manner.
- Reviews referral clinical content to ensure they are appropriate for admission to Home Health Agency based on agency formulary.
- Process new patients and coordinate initial insurance authorization.
- Ensure eligibility is correct, verify secondary insurances at time of referral.
- Complete welcome call to all new patient referrals and document via communication note findings in EMR.
- Maintain daily pending admit report and communicate with Clinical Managers/Directors, Administrator/Alt Administrator, Business Development, and both Nursing & Therapy Scheduling departments daily.
- Responds to the Agency and patient needs in a professional and creative manner.
- Participates in developing and maintaining efficient systems for intake process.
- Participates in coordinating care with business development, nursing supervisors, patients, and the interdisciplinary team.
- Refers patients and patients’ families to appropriate individuals/agencies for assistance with any health services that we as an agency are unable to meet.
- Effective and professional communication with Orders Management, Business Development, QA, and referral sources along with effective communication to patients, staff members, other healthcare professionals.
- Responds to the Agency and patient needs in a professional and creative manner.
- Establishes and maintains positive ongoing relationships with referral sources.
- Demonstrates commitment, professional growth, and competency.
- Participates in the Agency’s sponsored in-service training.
- Participates in the Agency’s QAPI program.
- Promotes the Agency’s philosophy and ensures compliance with all state and federal Intake regulatory requirements.
- All other duties as assigned.
