Licensed Vocational Nurse (LVN)
Marlora Post Acute Rehabilitation Hospital is looking for a dynamic Licensed Vocational Nurse to assist our clinical team and provide our residents with an exceptional care experience. The Licensed Vocational Nurse is a member of the patient care team responsible for providing direct nursing care under the supervision of a Registered Nurse. The Licensed Vocational Nurse is also responsible for following the patients Plan of Care, providing treatments and working collaboratively with the members of the team to help meet needs of our patients ensuring positive patient care outcomes.
Responsibilities
- Medication Pass
- Prepares and passes medications as indicated. Administer medications according to policy and procedure. Observe and document patient's response to pertinent medications;
- Ensures medications are documented and ordered in a timely fashion and in accordance with the Company’s policies and procedures.
- Contacts the attending physicians for required orders, as needed.
- Monitors side effects of medications as indicated.
- Provides pain medication interventions as ordered, including evaluation of interventions.
- Provides resident teaching regarding medication as required.
- Nursing Care
- Monitors condition changes and properly documents and follows-up as necessary.
- Reports labs and x-rays results, condition changes, and incidents, etc., in a timely manner to physicians and family members/responsible parties as needed.
- Maintains awareness of comfort and safety needs of patient.
- Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of patient.
- Assures residents are provided with good nutrition and with necessary fluids for hydration.
- Assures that documentation is accurate: Completed timely, weekly summaries are completed per schedule
- Ensuring that all nursing care is provided with privacy & dignity
- Timely reporting of change in resident’s condition to the nurse supervisor
- Assisting and supervising CNAs
- Monitoring residents that are at risk for falls
- Abiding with all facility policies and procedures
- Following Infection and Control policies
- Implement and follow resident’s plan of care
Experience:
- Skilled Nursing: 1 year (Preferred)
- New Grads are encouraged to apply
License/Certification:
- LVN (Required)
- CPR Certification (Required)
- Competency in Point Click Care (PCC) software is highly preferred
