SUMMARY:
Responsible for coordinating and directing the daily living activities for clients in our 24 hour residential programs. Person must be able to work direct care with our individuals on a regular basis and manage two locations with approximately 30 employees. Duties include managing the programs daily operations, client assessment, employee scheduling, training, and supervising, and assisting. On-call coverage is required.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Conduct annual assessments with clients and staff to identify client needs and prepare assessment summaries.
2. Coordinate schedules, reports, and team attendance for client's IPs and quarterly reviews.
3. Develop and plan methods of instruction and techniques for achieving individual client goals.
4. Direct schedule and coordinate comprehensive service plans for program clients and prepare required reports.
5. Monitor and provide care for the health needs of the clients, scheduling and attending health appointments as needed.
6. Contact schools and vocational sites to ensure the integration of services occurs.
7. Complete all required documentation and reports in accordance with Agency requirements.
8. Direct and coordinate transportation for clients to and from vocational and/or day programs, community outings or activities, and client family gatherings.
9. Assist clients and staff with meal planning, shopping, and food preparation in compliance with safe food handling procedures.
10. Maintain individual client's medical records, residential client records, financial records, and client's personal funds.
11. Ensure that the Executive Director, Clinical Director, DDS Case Manager, and any other appropriate persons are aware of all client's illnesses, accident, restraint usage, medication changes, etc.
12. Monitor and report all incidents or allegations of abuse, neglect, mistreatment, harassment, or threats of injury made by any client, staff member, visitor, other agency personnel, or family member.
13. Ensure 24-hour coverage is in place in accordance with the on-call schedule.
14. Assist, teach, and supervise clients in all aspects of daily living activities and individual client goals.
15. Effectively and promptly handle day-to-day issues that arise, being sure to follow Agency and DDS policy and procedures.
16. Provide information to the Executive and Clinical Directors regarding client, staffing, personnel, consultant, and residential concerns.
17. Maintain all required certifications (i.e. medication administration) and trainings to effectively perform job duties.
18. Perform any other duties as assigned or required by the Executive Director or designee.
SUPERVISORY RESPONSIBILITIES:
1. Assist in the hiring, training, evaluation, and personnel action needed to provide quality care to the clients.
2. Train direct care staff in the protection of client's rights.
3. Evaluate direct care staff job performance during probationary periods and annually.
4. Provide on-site supervision in compliance with the Administration of Medication to clients as approved by DDS regulations.
5. Coordinate direct care staff schedules to ensure the needs of the clients are met.
6. Provide training, as needed, or designated, to direct care staff.
7. Ensure all staff complete and attend the required trainings for their job duties.
8. Plan and organize programs to train staff in implementing programs and daily living activities with the clients.
9. Provide or complete required financial paperwork for clients in the program.
10. Provide or complete all required financial paperwork for the program.
TEAMWORK:
Attend required meetings and actively participate in in-services and trainings. Work cooperatively with the Clinical Director, Consultants, families, and other agencies in providing and implementing individual client goals. Work cooperatively with supervisory and other staff within the Agency and those from other agencies. Communicate effectively, professionally, and respectfully to all clients, Agency, DDS, community, and outside Agency personnel.
COMPETENCIES:
Successful completion of Medication Certification program, PMT, CPR, and First Aid training program, valid CT Driver's license with a satisfactory driving record, maintenance of all required certifications and trainings to effectively perform job duties.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
EDUCATION AND/OR EXPERIENCE:
Associate's Degree in Human Services or related field, two years' experience supervising programs or services for individuals with developmental disabilities, behavioral challenges, or medical issues, or the equivalent combination of education and experience.
LANGUAGE SKILLS:
Ability to read and interpret English documents such as safety rules, maintenance instructions, medical reports, and procedural manuals. Ability to write routine reports and correspondence in English. Ability to effectively present information and respond to questions from clients, families, regulatory agencies, and other employees.
PHYSICAL DEMANDS:
The employee is regularly required to talk and hear. The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls, and reach with hands and arms. The employee frequently lifts and/or moves up to 75 pounds and may be required to exert in excess of 100 pounds when needing to initiate physical restraint procedures (training provided). Specific vision abilities required by this job include close vision and the ability to adjust focus.
WORK ENVIRONMENT:
The noise level in the work environment is usually moderate. While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions.
