Manager of Care Management (RN) - HumanHire

We are seeking an experienced and dedicated Manager of Care Management (RN) to lead our client's care management team. In this role, you will oversee the coordination and delivery of care services, ensuring that patients receive comprehensive and effective care throughout their healthcare journey. You will work collaboratively with interdisciplinary teams to optimize patient outcomes, enhance care quality, and manage resource allocation.

• Location: Mineola, NY
• Salary: Up to $150,000/year plus bonus
• Work Model: hybrid - 3 days in office, 2 days remote

Responsibilities:

• Manage and mentor a team of care managers and case managers, fostering a culture of teamwork, accountability, and continuous improvement.
• Develop and implement care management strategies that facilitate seamless transitions of care and enhance patient satisfaction.
• Review and monitor care plans to ensure adherence to clinical guidelines, best practices, and regulatory requirements.
• Track and analyze key performance indicators to measure the effectiveness of care management initiatives and drive improvements.
• Collaborate with physicians, social workers, and other healthcare professionals to create comprehensive care plans tailored to individual patient needs.
• Serve as a liaison for patients and families, addressing concerns and ensuring access to appropriate resources and services.
• Provide ongoing training and support to care management staff to enhance skills and competencies in care coordination and patient engagement.

Qualifications:

• Bachelor’s degree in Nursing (BSN) required
• Current Registered Nurse (RN) license in NYS
• Minimum of 5 years of nursing experience, with at least 2 years in a care management or leadership role
