Position Statement

The Manager of Population Health will lead quality efforts in the ambulatory primary and specialty care practices. This role will center on the improvement of best-practice workflows and patient care outcomes through identification and implementation of system wide interventions that focus on quality, cost, and patient satisfaction.

Key Responsibilities:

• Acts as the primary for Health Program Manager and subrecipient of the Blueprint Grant. Works collaboratively with the Community Health Team Supervisor to ensure deliverables for grant funded positions including but not limited to: Medication Assisted Treatment (MAT), Pregnancy Intention Initiative (PII), and Self-Management Programs are met in a timely manner.
• Works with primary care leadership to disseminate key information regarding the implementation of workflow improvements that support NCQA, RHC, Blueprint, ACO, and/or commercial payer quality initiatives.
• Oversees annual NCQA accreditation submission in collaboration with regional QI facilitator and Clinical Performance Specialist.
• Prepares, submits, and oversees grant deliverables with data support from the Clinical Performance Specialist.
• Leads MHM quality and clinical improvement initiatives that work to promote effective care transitions, utilization of services, reduction in unnecessary readmissions and optimal outcomes.
• Reviews data and trends from reports generated by Clinical Performance Specialist, determines opportunities related to quality gaps, and develops/initiates improved workflows with clinical leadership.
• Supervises: Community Health Workers, Clinical Performance Specialist, Regional Self-Management Program Coordinator.

Knowledge, Skills and Abilities:

• Bachelor’s degree in health-related field required.
• History with grant application and administration, NCQA accreditation and submission experience, working knowledge of VT Blueprint for Health, and general quality and process improvement experience, preferably in a supervisory role.
