About the Role: As the MDS Coordinator, youll oversee the assessment process, ensuring accurate and timely completion of MDS forms in accordance with federal guidelines. Your responsibilities will include tracking Medicare residents, conducting concurrent MDS reviews, assigning diagnostic codes, and identifying opportunities to optimize reimbursement. You'll also play a key role in interdisciplinary team meetings, contributing to the facilitys strong survey history.
Key Responsibilities:
- Complete MDS items, care plans, and CAAs per PDPM and federal regulations.
- Ensure the accuracy of MDS entries before submission and conduct routine audits.
- Participate in interdisciplinary team meetings to communicate RAI/MDS information.
- Track facility CMI on a weekly and quarterly basis, ensuring compliance with all RAI/MDS protocols.

Requirements:
- NJ RN License
- 2+ years of clinical nursing experience in long-term care
- Expertise in RAI/MDS processes and reimbursement
- AANAC certification preferred
