Sapphire Nursing and Rehab at Goshen, is seeking an experienced full-time MDS Coordinator. The MDS Coordinator is responsible for overseeing the entire patient assessment process and ensuring that assessments and care plans are done accurately and in a timely manner. The successful applicant will be RAC - CT MDS certified and have knowledge of PDPM and OBRA regulations pertaining to MDS. The qualified applicant should be detail oriented and able to work independently in a manner that is accurate and concise. We are looking for experienced candidates but will train qualified candidates.

Ø We are currently paying a $4,000.00 signing bonus for this position (to be paid upon successful completion of 90 day probationary period)
This position requires on site work.
Requirements:
· Active RN license from the State of New York.
