About the Job
1. Completes relevant sections of the Minimum Data Set form as assigned, in accordance of
State and Federal guidelines.
2. Develop and update careplans to encourage residents to function at their highest practical level.
3. Participate in and gives inservices related to resident 's plan of care.
4. Coordinates care plan meetings and interacts with other disciplines to achieve care plan goals.
5. Document on resident's chart, relevant information.
6. Call physicians for orders in relation to triggers RAPS and care plan.
7. Attend all mandatory inservices, continually upgrades self in professional continuing educational programs and provides documentation of such to the Director of Nursing.
B. FUNCTIONS RELATED TO POLICY, PROCEDURES, AND ORGANIZATIONAL RELATIONSHIPS:
l . Assist in maintaining facilities policy and procedures and in meeting it's objectives as it relates to nursing care.
2 Responsible for reviewing the documentation of all disciplines as it relates to the MDS.
3. Keep superiors and subordinates informed as indicated to ensure and maintain quality resident care and good morale among staff members.
C. FUNCTIONS RELATED TO PHYSICAL ENVIRONlvlENT:
I. Participates in the evaluation of new equipment, supplies and techniques that may improve the resident's care.
2. Maintains a safe, orderly, attractive and clean environment forresidents.
3. Cooperate with other departments to maintain a safe and sanitary environment. 4. Maintains infection control practices according to facilities policies and procedures.
D. FUNCTIONS RELATED TO COST CONTROL:
I. Ensure that interdisciplinary Care Plans and PPS assessments are completed in time frame establish by State and Federal guidelines to ensure proper reimbursement.
2. Uses supplies and equipment judiciously; teaches subordinates proper and economical use of supplies and equipment.
3. Offer suggestions for more cost effective and economical use of supplies and equipment.
E. FUNCTIONS RELATED TO PUBLIC AND PERSONNEL RELATIONSIIlP:
1. Maintains good working relationships and utilizes good judgment with all staff members and disciplines to achieve work expectations.
2. Fosters and maintain cordial relationship with the residents, their families and friends, interested persons or groups in the community.
3. Keeps abreast of the new trends in nursing and increase his/her clinical expertise by reading current professional journals and consulting clinical reference materials available to him or her.
4. Has complete control of the English language.
5. Observe and respect resident's rights and dignity at all times.
F. OTHER FUNCTIONS:
I. Participates in continuous quality improvement (CQI) and quality assurance monitoring programs when assigned by DON.
2. Assumes additional responsibilities assigned by DON or Designee.
