We are seeking an experienced MDS Coordinator to be a key player in our efforts to ensure compliance and optimize reimbursement through accurate and timely completion of the Minimum Data Set (MDS).
Position Overview:
As the MDS Coordinator, you will play a critical role in our facility’s success by managing the MDS process in accordance with federal and state regulations. You will work closely with our interdisciplinary team to assess resident care, ensure proper documentation, and oversee the accuracy of resident assessments to ensure optimal reimbursement and compliance.
Key Responsibilities:
- Oversee the completion of MDS assessments in a timely and accurate manner
- Ensure compliance with federal and state regulations, as well as facility policies
- Collaborate with the interdisciplinary team to develop individualized care plans based on comprehensive resident assessments
- Coordinate and facilitate care plan meetings to ensure resident needs are met
- Monitor and audit documentation for accuracy, completeness, and compliance with CMS regulations
- Train and educate staff on MDS processes, regulations, and documentation requirements
- Optimize facility reimbursement through effective management of MDS data submission and quality reporting
Qualifications:
- Licensed RN or LPN in [Insert State]
- Minimum of 2 years of experience as an MDS Coordinator or in a related role within a long-term care or skilled nursing facility
- Comprehensive knowledge of MDS 3.0, RAI process, and care planning
- Strong understanding of Medicare, Medicaid, and reimbursement processes
- Excellent attention to detail, organizational, and time-management skills
- Strong interpersonal skills with the ability to communicate effectively across disciplines
- Knowledge of PointClickCare or other electronic health record (EHR) systems is a plus
