Job Summary: The MDS (Minimum Data Set) Coordinator is responsible for overseeing the MDS assessment process in long-term care facilities. This role ensures compliance with federal and state regulations, maintains accurate patient records, and coordinates with interdisciplinary teams to develop and implement individualized care plans.
Key Responsibilities:
- Assessment and Documentation:
- Complete and update MDS forms for all residents in the facility.
- Perform clinical assessments to gather comprehensive information on residents' health status.
- Ensure accurate and timely submission of MDS data to maintain compliance with Medicare and Medicaid requirements.
- Care Planning:
- Develop and update individualized care plans based on MDS assessments.
- Coordinate with nursing staff, physicians, and other healthcare professionals to implement care plans.
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
- Compliance and Quality Assurance:
- Ensure all MDS documentation meets federal and state regulations.
- Participate in quality assurance and performance improvement activities.
- Conduct audits and reviews of MDS documentation to ensure accuracy and completeness.
- Communication and Collaboration:
- Facilitate interdisciplinary team meetings to discuss resident care and progress.
- Communicate with residents and their families to explain care plans and address concerns.
- Serve as a resource for staff regarding MDS processes and regulations.
Qualifications:
- Education:
- Registered Nurse (RN) preferred or Licensed Practical Nurse (LPN) with a valid state license.
- Certification in MDS (e.g., RAC-CT) preferred.
- Experience:
- Minimum of 2 years of experience in a long-term care setting.
- Previous experience with MDS assessments and care planning.
- Skills:
- Strong knowledge of MDS 3.0 and federal/state regulations.
- Excellent organizational and time management skills.
- Effective communication and interpersonal skills.
- Proficiency in electronic health records (EHR) systems.
Work Environment:
- Primarily works in a long-term care facility.
- May require occasional evening or weekend shifts to meet the needs of residents.
