Attention all MDS Coordinators! A leading Nursing Home in your area is seeking a highly organized and detail-oriented individual to join our team as an MDS Coordinator.
As the MDS Coordinator, you will play a critical role in ensuring that our residents receive the highest quality of care and services. You will be responsible for coordinating and completing all aspects of the Minimum Data Set (MDS) assessment process, including conducting assessments, creating care plans, and ensuring that residents receive the appropriate level of care and services.
Responsibilities include:
- Coordinating and conducting the MDS assessment process for all residents, including conducting assessments, creating care plans, and ensuring that residents receive the appropriate level of care and services
- Collaborating with interdisciplinary teams, including nursing, rehabilitation, and dietary, to ensure the accuracy and completeness of the MDS assessments
- Ensuring that all MDS assessments are completed in a timely and accurate manner and in accordance with regulatory requirements
- Monitoring and analyzing resident outcomes and using data to identify areas for improvement in care and services
- Providing training and education to staff on the MDS process and regulatory requirements
- The ideal candidate will have:
- An RN license and experience in long-term care
- Strong knowledge of the MDS assessment process and regulatory requirements
- Excellent organizational and analytical skills
- Strong communication and interpersonal skills
- Ability to work effectively as part of a team and to build positive relationships with residents, families, and staff
