About the Job
Allegria Village, a senior living community in Dearborn, is currently hiring a full-time Minimal Data Set (MDS) Coordinator. Some responsibilities to include:
Job Responsibilities:
- Responsible in developing treatment plans appropriate for patients that comply accordingly to MDS guidelines.
- Completing various MDS paper works; submit them on time and make certain it meets with the current guidelines and regulations as these regulations may vary each year.
- Further assess how residents with skilled nursing along with long term healthcare facilities function, and the necessary steps that need to be taken in order to support them.
- Collaborative work with other medical staff and run assessment care plan meetings.
- Educating and coordinating clinical members of the staff as to the proper use of its system.
- MDS coordinator can also perform some nursing tasks if needed.
- Develops a working knowledge of state regulations and ensures compliance through supervising and coaching team members.
- Assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents.
- Speaking to residents and ensuring their wellbeing through regular checks.
- Following up on complaints and incidents reported by residents.
- Coordinating with other key stakeholders to ensure proper care implementation.
- Reporting findings and making recommendations to management.
- Other duties as assigned
Qualifications:
· Minimum 1 year experience with MDS in Skilled Nursing
· A solid understanding of data collection, reporting, and analysis.
· The ability to draft and create thorough care plans for residents.
· Exceptional analytical skills.
· An excellent work ethic and understanding of MDS procedures.
· Superb organizational and interpersonal skills.

· Great communication and people skills
· Excellent organizational and problem-solving skills
· Must be able to read, write, speak, and understand the English language.
· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Education/Certification
·Registered Nurse (RN) from Accredited School and in compliance with regulations of the state
