Job Description Summary:
The MDS Department is responsible for ensuring the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of the facility
The objective of MDS coordinators are to promote the physical and emotional well-being of nursing facility residents, create and review resident care plans and analyze the financial coverage to assure resident is getting what they need.
MDS will need to be familiar with the Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic interviews.
The MDS covers such areas as the patient's mood, behavior patterns, cognitive ability, and nutrition needs.
Information from these assessments provides a scale for the nursing team to formulate individualized care plans that include support from social services, dietitians, rehab specialists, and medical staff.
MDS coordinators then implement and monitor these care plans to ensure their effectiveness.
They must also make sure all strategies comply with Medicare requirements and ethical standards.
Qualifications and Skills
Requirements/Qualifications:
Graduate of an accredited School of Nursing (RN)
Current/active license for Registered Nurse
MDS coordinator certification
Clinical Experience in a nursing facility
Demonstrated clinical assessment skills
Knowledge of Medicare and Medicaid regulations and coverage guidelines as related to skilled nursing.
Required License/Certifications: RN License and RNAC Certification
