Position Summary: An experienced registered nurse with the knowledge and skills to coordinate and oversee all functions of the MDS and resident care assessment process to assure compliance with federal and state requirements, and to certify timely completion of MDS assessments.
Location Overview: This position is located at University Crossing, our Joint Commission certified, three story, 82,000 square-foot, 111 bed facility. This facility serves patients with both short and long-term rehabilitation needs.
Job Responsibilities:
- Maintains confidentiality of patient information.
- Adheres to policies and procedures specific to patient rights.
- Maintains a clean and safe environment; identifies and reports hazards.
- Meets all mandatory In-service requirements.
- Observes infection control procedures.6.
- Adheres to policies/procedures specific to universal precautions when delivering patient care.
- Observes all facility safety policies and procedures.
- Adheres to policies regarding attendance, conduct, grooming and dress code.
- Promotes a positive image of the facility to outside agencies and the public.
- Communicates and interacts appropriately with co-workers.
- Completes annual equipment and skills checklists.
- Exhibits excellent service to resident, visitors, physicians, and co-workers.
- Shows courtesy, compassion and respect.
- Exhibits values of the organization.
- Schedules or oversee scheduling of all resident MDS assessments to assure timeliness.
- Coordinates and oversees all functions of the MDS assessment process to assure compliance with federal and state requirements.
- Participates in the pre-admission process to determine potential residents’ level of care.
- Participate in resident level of care determination upon admission.
- Participates in tracking changes in resident level of care to assure timely scheduling of MDS assessments.
- Familiar with residents’ condition and care needs.
- Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
- Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as required.
- Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure information is included.
- Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
- Maintain confidentiality of resident and facility information.
- Complete reports and assignments accurately and adhere to established time schedules.
- Establish open lines of communication between all nursing personnel.
- Observe infection control procedures relating to nursing services.
- Observe all facility safety policies and procedures.
- Come to work as scheduled and consistently display dependability and punctuality
- Consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families and consultants.
- Observe all facility policies and procedures.
- Observe Residents’ Rights policies at all times.
- Other duties as assigned by the Management.
Job Qualifications:
- Graduate of an accredited school of professional nursing, licensed as a registered nurse in the State of Florida with at least three experience in skilled nursing.
- Basic Life Support (BLS) certification required.
- BSN or Baccalaureate in related field and CRRN nursing preferred.
