Responsibilities
Coordinate home visits with prospective and existing participants to ascertain level of care. Act as a liaison between the Program of All-Inclusive Care for the Elderly (PACE) and state agencies.
Responsibilities:
+ Submit annual state mandated Medicaid eligibility renewal packets and supporting documents to agencies to maintain participant's Medicaid status and PACE eligibility.
+ Assist with submitting Pre-Admission Screening & Resident Reviews (PASRR) and annual Pre-Admission Evaluations (PAE) recertification as needed.
+ Utilize participant financial documents to complete Medicaid state applications for approval.
+ Monitor, respond, and communicate application status to participant and interdisciplinary team.
Requirements
Licensure / Certification / Registration:
+ Practical/Vocational Nurse preferred.
+ Registered Nurse preferred.
+ Social Worker preferred. Bachelors or Masters Accepted.
Education:
+ High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
+ Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
