NaphCare is hiring an experienced Medical Discharge Planner just like you to join our team at the Wayne County Jail located in Detroit, MI. Pay rates range from $22.00-$30.00 per hour based on employment status, shift, specific facility, site needs, licensure, and experience.

Responsibilities for Discharge Planner:

• Conduct rounds with physicians to meet with patients to discuss placement orders
• Assist patients with a specific plan for medication management
• Include patient in discharge planning
• Assist patients with the identification of a support network, instructions for future care and follow-up
• Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors

Qualifications for Discharge Planner:

• Bachelor’s Degree in a human service field
• Minimum of 1-year experience in a clinical setting, with correctional healthcare experience preferred
• Knowledge and experience in the development of discharge plans and coordination with community service providers
• Valid CPR card
