Responsibilities:
- Medical Social Worker will coordinate and provide social services for patients of all ages in their homes
- Advocating for patients who are working through psychological, health, family, and financial struggles.
- Assessing the mental and emotional stability of the patient by understanding the social and financial condition of the patient along with their family.
- Coordinate and collaborate with health-care workers to provide optimal health-care services to the patient.
- Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of Clients.
- Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
Required Skills/Abilities:
- One year Traveler Experience
- One year Home Care experience.
- Once Year of social work experience
- Current licensure in the state of Massachusetts
- Reliable, independent means of transportation to make home visits
