Position Overview: Longevity Health is seeking a dynamic Medicare Advantage Account Manager for a field-based role, reporting directly to the Market Vice President. This role is crucial in supporting member care and managing relationships with Skilled Nursing Facilities (SNFs). The successful candidate will coordinate care, meet enrollment goals, and ensure high-quality, patient-centered care for our I-SNP members.
Key Responsibilities:
- Facility Coordination: Oversee plan functions for assigned SNFs, ensuring high-quality and cost-effective care in collaboration with facility staff and management.
- Primary Contact: Act as the main liaison between facilities and Longevity Health, collaborating with facility management to ensure quality services and effective plan administration.
- Regular Visits: Conduct at least monthly visits to assigned SNFs to maintain effective communication and address any issues.
- Contract Management: Manage SNF contractual relationships, ensuring accurate administration of contract terms and leading multidisciplinary teams.
- Operational Issue Resolution: Identify and resolve operational issues to maintain positive long-term relationships.
- Data Review & Reporting: Perform monthly data reviews, present educational topics, and manage communications related to HEDIS reporting and other audits.
- Sales & Enrollment: Achieve monthly enrollment goals through lead generation, referral development, and sales presentations. Work with the Market VP to develop and execute facility-specific strategies.
- Provider Relations: Support contracting efforts and build relationships with key providers, including IPAs, hospitals, and specialty services.
- Program Implementation: Oversee I-SNP program implementation in facilities, ensuring compliance and readiness for launch.
- Documentation & Compliance: Maintain accurate member records, comply with State and Federal regulations, and uphold high workplace behavior standards.
Qualifications:
- Education: Bachelor’s degree required.
- Experience: Minimum of 3 years in the healthcare industry focusing on provider relations, contracting, and business development; at least 2 years of sales experience preferred.
- Skills: Strong communication and presentation skills, project management and coordination abilities, and a solid understanding of managed care and contractual arrangements.
- Licensing: Health Insurance Producer License (or ability to obtain within 60 days). Valid state driver’s license and proof of auto insurance required.
- Other: Proficiency in Microsoft Office Suite, willingness to travel within the assigned territory, and comfort working in a Skilled Nursing Home environment.

Longevity Health is a clinical services company and national Institutional Special Needs Plan (ISNP) founded with the purpose of optimizing the quality of life for people in long-term care by focusing on their individualized health needs.
Launched in 2018, our ISNP was designed to address the significant unmet needs of nursing home residents by establishing collaborative, value-based partnerships with long-term care facilities.
