Job Summary
The Memory Care Coordinator is a strong leader that has passion in living life to the fullest and can pass that mentality onto the residents and team members through great nursing care.
Essential Responsibilities
- Provides services (ADL's and Medication Supervision) in accordance with specific written orders from a physician and under the supervision of an RN.
- Records all services given on appropriate forms or POC device.
- Obtains vital signs when requested and records.
- Reports and documents pertinent information and prepares progress notes.
- Greets new residents making a special effort to include and engage them in activities and introduce them to others in the community with similar interests and backgrounds.
- Promotes a positive, respectful, and supportive environment for all residents, team members, families and guests.
- Maintains confidentiality of all resident information and resident records. and safeguards the resident's private property.
- Maintains a safe work area free from hazards of fire, spills, chemicals and equipment. Reports any problems to the Health and Wellness Director and/or President.
- Attends all Community required training sessions to maintain professional competencies.
- Performs other duties as assigned.
- Communicate effective with team members assigned to different shifts.
- Expands personal development, skills and knowledge of activity programs as it relates to seniors with moderate to severe memory impairments in a long-term care setting.
Required Knowledge, Skills and Abilities
- First aid and CPR certification
- Demonstrate good communication skills, to be able to work with residents, families, physicians, and team members.
- Show passion and dedication to the well-being and potential of each resident.
Education and Experience
- Nursing Degree
- Has two years of experience in a social or recreational program within the last five years, one year of which was full-time in an activities program in a healthcare setting.
- Have 2 or more years of direct engagement and interaction with Memory Care residents
- License to practice in State.
- CPR Certification
