About the role
The Memory Care Program Coordinator is responsible, in collaboration with the Memory Care Program Manager/ Executive Director for planning, developing, organizing, and implementing a person-centered care program and schedule meaningful events for residents with Alzheimer's and other related types of dementia. The Memory Care Program Coordinator is responsible for completing the Resident Social History Form to assess residents to help support personalized programming through a monthly calendar that focuses on the needs of the resident including holistic approaches for social support. In partnership with the Memory Care Program Manager/ Executive Director and Resident Care Director, this role will be a resource for educational opportunities for families and continual staff training. This individual also assists with identifying behavioral challenges and programming to help meet our residents' needs.

What you"ll do
- Responsible to complete a Resident History form to help determine what level of programming a resident can successfully achieve and develop meaningful daily activities based on this information.
- Responsible for the developing and completing a monthly calendar based on current residents' interests and needs.
- Actively lead the structured activities noted on the calendar as well as unstructured moments during the day.
- Responsible for the oversight and managing the Memory Care Assistant.
- Motivate and encourage Resident Care employee's participation and involvement through coaching and leading by example, in partnership with the Resident Care Director.
- Responsible for being present and supporting the Resident Care Technicians during the dining program, and supporting the resident's social needs during mealtime, as well as supporting menu choices and quality food presentation.
- Responsible to partner with the Resident Care Director in identifying behavioral identifiers and educate on meeting the unmet needs.
- Responsible for alerting the Resident Care Director/Executive Director of any behavioral, physical or emotional changes in our memory care residents.
- Responsible for partnering in organizing and leading support and/or education groups for families as well as participating in continual dementia education for employees.
- Find opportunities to give the resident choices throughout the day and allow them to be as independent as possible.
- Lead, engage and motivate residents for program participation.
- Provide verbal encouragement and support to residents.
- Prioritize tasks to ensure optimum services to residents as requests and needs change.
- Communicate with the Executive Director & Resident Care Director when a resident's need has changed.
- Communicate to other departments as residents' needs arise during the shift.
- May perform other duties as needed and/or assigned.
Requirements & Qualifications
- High School Diploma or equivalent required. Associate or Bachelor"s Degree strongly preferred in Social Service, Business, or a field related to the position.
- Minimum of 3 years' experience in Assisted Living/Memory Care preferred.
- Adaptability and strong organization and time management skills.
- Excellent resource management, computer, record keeping, interpersonal, decision making, and verbal and written communication skills.
- Patience and compassion when dealing with others, especially those who are sick, injured, elderly, or from diverse backgrounds.
- Flexibility to work when needed, especially evenings, weekends, or early mornings.
- Willingness to adhere to and enforce all safety policies and procedures.

- Prolonged periods of sitting at a desk and working on a computer.
