Position Description
Summary of Responsibilities:
Under the general direction of the Blended Case Management Supervisor, this person plays a critical role in coordinating and advocating for the mental health needs of the individuals served. The mental health case manager provides excellent customer service is proficient in the use of a personal computer and is adept at handling multiple tasks at one time. The mental health case manager plays a pivotal role in promoting the holistic well-being of individuals struggling with mental health issues, aiming to enhance their quality of life and facilitate their journey towards recovery and self-sufficiency. Duties are conducted in accordance with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company.
Essential Functions:
Assessment & Evaluation.
Conducting initial assessments to determine the needs of clients, including their mental health status, living situation, and any other support systems they may have in place.
Treatment Planning.
Coordinating with individuals served, their families, and other team members to develop comprehensive treatment plans tailored to the specific needs and goals of each individual.
Referral Coordination.
Connecting individuals with appropriate mental health services and resources, including therapists, psychiatrists, support groups, and community programs that can facilitate well-being.
Monitoring & Support.
Regularly monitoring goal progress and providing onboarding support, guidance, and encouragement to ensure that individuals are following through with their treatment plans and making necessary adjustments as needed. Oncall and weekend hours are required.
Crisis Intervention.
Responding promptly and effectively to any mental health crises that may arise, ensuring the safety and well-being of individuals served while implementing appropriate interventions and strategies to manage and de-escalate the situation.
Advocacy.
Serving as an advocate for individuals by ensuring their rights are upheld, their voices are heard, and they have access to the necessary resources and services to improve their overall quality of life.
Documentation.
Maintaining accurate and up-to-date records of individual progress, treatment plans, and any significant changes or developments, while also adhering to legal and ethical standards of confidentiality.
Continuous Professional Development.
Staying updated on the latest developments in the field of mental health, including new treatment modalities, interventions, and resources, through continuous education and training to ensure the delivery of high-quality and evidence-based care.

Education and Experience:
Bachelor's Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field. Or, 12 credit hours in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or a related field AND 2 years experience.
MH/DD experience preferred
Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver's License.
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, ability to travel throughout the region, travel may be in adverse weather conditions, must be able to lift to 15 pounds.
