Case Management
Essential Duties & Responsibilities include:
- Assist in screening and selection of referrals in cooperation with Unit Manager
- Assume supervision of staff and management of program in absence of Unit Manager
- Provide direct care support to clients in area of, but not limited to, medical, daily living skills, money management, personal growth and development, vocational training, socialization, community integration, self preservation, community awareness and safety
- Assist in development of ISP and implementation of services
- Ensure clients' records are current
- Complete necessary paper work for IRS' charts as assigned
- Make appropriate referrals to medical or mental health needs of clients
- Attend Alternatives, Inc.'s General Staff meetings and Unit meetings
- Participate as a team member in planning, implementation and evaluation of services provided by Alternatives, Inc.
- Monitor clients' progress to ensure placement in the most appropriate and least restrictive setting
- Lead by example when working with clients and co-workers
- Assist clients in transportation for medical, dental, work and/or day program, and therapy appointments
- Maintain positive and productive relationships with inter-agency departments, external agencies, day support providers, community programs, etc.
- Complete monthly reports as assigned and submit to Unit Manager in accordance with assigned deadlines
- Assist Unit Manager in adherence to budgetary guidelines
- Assist in scheduling, handling call-outs and responding to emergencies as assigned
- Ensure security and confidentiality of all assigned Alternatives' records
- Investigate and remain informed of community resources and activities of IRS
- Follow all policies and procedures relative to this position
- Cooperate with Alternatives, Inc. in all inspections or investigations
- Adhere to person-centered philosophy, agency policy, and procedures and confidentiality
- Maintain current training records and assist Unit Manager with overall program records
- Maintain current driver's license and clean driving record
- Perform necessary tasks for clients
- Assist clients with maintenance and upkeep of apartments
- Maintain contact with outside agencies providing services to clients
- Work to provide socialization opportunities for clients
- Act as consultant to assess both physical and mental health care facilities to help meet the needs of
clients
- Remain informed of literature related to services provided by Alternatives, Inc.
- Handle emergency situations as they arise in a calm, professional manner
- Ensure operational effectiveness in emergency situations by providing services at Alternatives, Inc.'s program sites as assigned
- Strong interpersonal skills required

Skills Required BA & experience REQUIRED. Must possess valid NJ/NY/PA driver's license **In order to be eligible for employment, applicant must not have 7 or more points on their driving record and no DUIs within the last 3 years.**
