Mental Health Clinician / Social Worker - Tandym

A multidisciplinary Healthy Families New York Home Visiting Program is looking for Clinician to join their team in Syracuse NY!

About the Opportunity:

• M-F Daytime hours (flex).

• Office/remote/field. 1 office day and must meet team every day remotely. 3 office days 2 remote. Clinical supervision provided.

Responsibilities:

• Support the clinical team in evaluating the needs of youth and families by utilizing therapeutic interventions to foster positive outcomes.
• Collaboratively design, execute, assess, and modify individualized care plans as part of a multidisciplinary team, addressing each client's specific needs.
• Coordinate and facilitate connections to Hillside programs and available resources provided by the care team.
• Offer individual, group, and family therapy based on client requirements. Partner with informal supports, community organizations, and mental health professionals to ensure clients receive necessary services.
• Maintain accurate and timely documentation, including but not limited to clinical evaluations, treatment plans, and progress reports. Serve as the main point of contact for youth treatment plans.
• When necessary, provide transportation for youth to appointments, meetings, or other engagements.
• Uphold all policies, procedures, and relevant government regulations.
  
  

Requirements:

• Master's degree in Social Work or Mental Health Counseling with a valid NYS Professional license.
• Minimum of 1+ years of relevant clinical experience working with youth and families facing mental health challenges.
• Thorough understanding of provided services, strong advocacy skills, and knowledge of payment processes.
• Must possess a valid driver's license and meet agency driving standards.
• Licensed or permit eligible upon hire.
