About the Job
Duties and Responsibilities:
- Provide individual, group and family counseling to youth and their families.
- Complete psychosocial assessments at time of intake to assist in determination of appropriate educational and socio-emotional services and supports.
- Provide treatment planning and case coordination of clients.
- Provide clinical case management services to members and their families.
- Provide crisis intervention, peer mediation, and conflict-resolution services.
- Determine and implement appropriate therapeutic interventions to effect change in problematic behavioral and social interactions.
- Collaborate with teachers and other HMHS support staff to implement strategies that support student learning.
- Identify academic and behavioral challenges through analysis of factors that are potential barriers to academic success including, home, school, and community.
- Serve as a liaison between HMHS and students' families/guardians to positively promote collaborative processes in educational planning.
- Support students with post-secondary educational planning and supporting students with college selection and application.
- Maintain clinical records and submit appropriate documents for statistical reporting with adherence to program standards.
- Conduct classroom observations to identify and implement appropriate interventions for behaviors impacting learning for self and/or others.
- Serve on attendance team to monitor and address issues of chronic absenteeism and tardiness.
- Provide outreach to families/guardians to assist in overall engagement in the educational process.
- Locate and mobilize community resources including community referrals to support students and families.
- Conduct psycho-educational and social-skills groups on identified areas that will help foster a positive learning environment for all HMI and HMHS young people.
- Serve as a liaison between HMHS and HMI to bridge and promote broad menu of programming and support services.
- Participate in professional development trainings on issues related to social-emotional-environmental factors that may be barriers to students' learning.
- Attend counseling team meetings, individual and clinical supervision groups, Youth Services Department and other team meetings as part of comprehensive program support team.
- Promote and model positive school culture with a focus on a no bullying approach to safe space initiatives.
- Partner with community agencies and assist in fostering communication between schools, parents/guardians, and community leaders.
- Support after-school program staff with general duties as assigned.
- Other duties as assigned.
Knowledge, Skills and Abilities Required:
- Current LMSW license with opportunity to obtain LCSW and SIFI certification. Counseling degrees with opportunity to obtain licensure are welcomed.
- Experience working with at-risk adolescent and LGBTQ populations and their families.
- Strong familiarity with the NYC DOE, 100ACS services, and surrounding social service agencies and resources.
- Ability to exercise independent judgment when appropriate; strong decision-making skills.
- Strong oral and written communication skills.
- Computer Skills: Strong with proficiency in Microsoft Word and Outlook; familiarity with spreadsheet/database applications.
Additional Desirable Qualifications:
- Knowledge of working within a systems, family, and/or positive youth development approach in the treatment of adolescents.
- Knowledge of cultural competency issues.
- A self-starter who is creative and flexible.
- Able to work flexible work hours.
