Job Summary
The counselor works with individuals or groups to explore how emotions, experiences, and relationships affect thoughts, feelings, and behavior and to define goals, develop a plan of action and utilize solution-focused strategies to help patients identify barriers, overcome challenges, and improve function.
Job Responsibilities
Conducts patient assessments and establishes therapy treatment plans and goals.
Provides direct patient/family care through biopsychosocial assessment and evidenced-based interventions.
Coaches, counsels, and conducts therapy sessions for patients in individual, and groupformats.
Promptly documents and maintains confidential records of observations and treatment.
Evaluates progress and effectiveness of therapeutic interventions through routine outcome assessment and adapts treatment strategies and goals as appropriate in collaboration with the patient.
Provides patient with information regarding resources and therapeutic activities in the community
Works directly with the patient family, physician, and community providers to ensure continuum of care and clinical communication from the initiation of treatment to the time of discharge.
Other duties as needed.
Requirements
Clinical Privileges: Counselor shall apply for and maintain clinical privileges in good standing at the Hospitals as a non-physician practitioner throughout the term of this Agreement, in accordance with the Applicable Requirements.
Licensure: Program Status: Counselor shall continue to hold an unlimited license to provide applicable mental health services in Indiana and shall comply with those laws, regulations, and standards of professional conduct governed by the appropriate Indiana State licensing board
Counselor shall participate as a provider in good standing in the Medicare and Medicaid health benefits programs.
Education Requirements
Minimum: Master’s Degree from an accredited college or university in a mental health field; psychology, counseling, social work, or related field required.
Experience Requirements
2 or more years of experience providing mental health treatment
Preferred: Clinical license required for reimbursement.
Preferred experience in medical setting.
License / Certification
State Issued License – Valid LMHC or LCSW Indiana license in good standing by time of hire.
CPR
Basic Life Safety certification from AHA- American Heart Association
Education

Bachelors or better in Social Work
