We areseeking aMental Health Practitionerto join our Safe Harbor team. This is a full-time, weekend position working 7am - 7pm.This position will primarily be located at our Crisis Bed facility. in Brainerd, MN.
Essential Functions include:
- Complete required intake information with individuals, including the consent for treatment, HIPAA Practices/Confidentiality, ROI, Emergency Contacts, Property Inventory, Orientation to property and program, policies and procedures, MNits verification, and insurance notification
- Inventory medications and complete the Medication Administration Record (MAR) when the RN is not on site
- Adiminister medications in accordance with agency medical procedures
- Complete the following assessment, Individual Abuse Prevention Plan (IAPP), Immediate Needs/ Individual Treatment Plan (INA/ITP), and substance use screening and assessment (CAGE-AID).
- Complete a PHQ-9 and Columbia-Suicide Severity Risk Scale (C-SSRS) with each individual at intake. Complete daily thereafter if PHQ-9 score is <9
- Be familiar with every individual's treatment plan and goals
- Update and complete ITP review at least one time weekly or when ITP goals are met
- Daily, exchange information necessary to carry out an individual's treatment plan, including updates and revisions to the INA/ITP and IAPP
- Provide crisis intervention and social conflict resolution as necessary to assist individuals who are experiencing mental health symptoms
- Conduct Integrated Dual Diagnosis Treatment, Illness Management, and Recovery sessions as scheduled using the developed curriculum
- Document each individual daily. Record attendance of all groups in a progress note
- Coordinate the discharge plan with individual and necessary support people. Develop the discharge summary and fax it to appropriate supports after discharge (pending valid ROIs).
- The individual and the referent complete the discharge satisfaction survey at discharge request. Follow up with the individual at 30 days to complete a telephone survey
- Assist with meal preparation and cleanup when needed and encourage individuals to assist as well
- Transport individuals as needed to appointments
Qualifications include:
- Must qualify as a mental health practitioner as defined by the Minnesota Department of Human Services in one of the following ways:
- Has at least 2,000 hours (approximately two years working full-time) of supervised work experience in the delivery of mental health services. OR
- Holds a master's or other graduate degree in behavioral sciences or related fields.OR
- Holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields.
- Valid Driver’s License and ability to operate a motor vehicle
- Must successfully pass a MN DHS background check prior to hire
- Current CPR and First Aid Certification is required. Training will be provided if not currently certified.
- Flexibility in scheduling as this is a 24/7 facility
