for our SASH - Support and Services at Home program Providence, RI.

As an active member of the Support & Services at Home (SASH) team, The SASH Mental Health Specialist (MHS), will be expected to work collaboratively with the SASH Coordinator and Wellness Nurse (RN) in providing whole-person, trauma-informed care coordination and support targeted to individual participants as well as the SASH population as a whole. Specific interventions by the SASH MHS will be flexible to meet the needs of program participants identified as high risk for social isolation, suicide ideation, anxiety, depression and in need of other emotional wellness support.
Qualifications for the SASH Mental Health Specialist:
- Bilingual English/Spanish Proficiency
- Master's degree in counseling, social work, psychology, or closely related field Or Bachelor's degree in social work or related field with at least 2 years' experience in behavioral health. Will consider previous behavioral health internship experience.
- Experience in service provision of trauma-informed care, and behavioral health needs of older adult population.
- Knowledge of and demonstrated experience with neurodegenerative conditions a plus.
- Knowledge of and demonstrated experience with best practices and state and federal laws related to elder care, health in housing, mental & behavioral health, for at-risk populations.
- Ability to assess, deliver, and evaluate direct service programming.
- Master's degree in counseling, social work, psychology, or closely related field Or Bachelor's degree in social work or related field with at least 2 years' experience in behavioral health. Will consider previous behavioral health internship experience.
- Lived experience strongly encouraged
- Experience working with older adults and/or disabled required
- Trauma-informed care delivery
Essential Responsibilities for a SASH Mental Health Specialist:
- Individual therapeutic support provided in-person, by phone and through telehealth and other psychosocial support.
- Assessment, screening, and triage services to help identify unaddressed needs and to assist individuals in connecting to clinician and/or other community resources and providers.
- Work in collaboration with SASH team to coordinate care.
- Along with the SASH team, develop and implement group-based, educational programs on topics relating to mental health, aging, and overall emotional wellness and other expressive needs.
- Contribute to and directly participate in additional support programming as assigned.
- Facilitate on site, educational and support groups the promote emotional wellness.
- Assist the agency in assessing and providing effective, equitable, understandable, and respectful quality care and services that are responsive to diverse cultural beliefs and practices, preferred languages, mental and behavioral health literacy, and other communication needs.
- Additional Direct Services may include conflict mediation sessions; support with completing Advance Directives and support with housing stability.
- Care coordination to include regular consulting meetings with SASH staff, weekly SASH team meetings, and outreach to primary care, psychiatric, and other community partners and service providers.
- Maintain client confidentiality and conduct self in accordance with professional and ethical standards
- Provide consultation to SASH Coordinators and Wellness Nurses addressing challenges confronted by SASH participants with mental illness, substance misuse and other social, behavioral, or emotional concerns.
- Utilize SASH data management system (s) to collect and present outcome data for the program.
- Maintain records/up-to-date documentation, including but not limited to release and authorization forms, assessments, treatment plans, progress notes, discharge summaries, and external records.
- Accurately completes timesheets and expense reimbursements on a timely basis.
